New Patient Information

Prior to your first visit, please complete our New Patient Registration Form, a Patient Health History Form, and our Smile Evaluation Form. You may complete the forms and submit them on-line, or you may print them and bring to your first visit. Alternatively, you may print the forms and complete them in ink and bring to your visit.

Your first visit

When you come into our office you will be greeted by one of our staff who will review your registration form and medical history form. An assistant will escort you to the examination area and necessary x-rays will be taken. Dr Borrello will then discuss with you the dental challenges you've had in the past and what your specific fears are. In most instances we will arrange to take care of your very pressing problems first and a consultation will be scheduled to review all your dental needs and finances. Your dental insurance will be reviewed as well.
